Here’s what to expect when you come see us — and how to make the most of your appointment.
What to bring
- A photo ID and your insurance card
- A list of medications you take, with doses
- Any recent imaging (X-rays, MRI, CT) or reports related to your foot or ankle
- The shoes you wear most — they often tell part of the story
- Your questions, written down if that helps
Patient forms
New-patient forms will be available for download here soon. For now, please arrive a few minutes early and we’ll take care of the paperwork together at the office.
Insurance
We work with most major insurance plans. Coverage varies by plan and procedure, so the fastest way to be sure is to call the office — we’ll verify your benefits before your visit.
